To change the SIM card format

To get a SIM card in the format of micro- and nano-SIM, you do not need to make an application - contact the nearest Vodafone store


If the number is lost

To block the SIM card and get a duplicate, you need to compose the application in one of the following ways:

  • contact the Online Service Center employee;
  • Call the operator at 111 (free of charge from Vodafone mobile in Ukraine) or 0 (800) 400 000 (for subscribers of other operators (free of charge from any number in Ukraine).

To confirm that you are the owner of the SIM card, you have to provide the codeword or PUK code (indicated on the plastic holder).       

If these data are missing, please provide additional information:  

  • Two phone numbers, you recently dialed out or sent SMS to;
  • he amount on the account and the amount of the last account replenishment or the date of the last action by the number.         

Restoring a SIM card can be done by messaging only an adult. To fill out the application for restoring, provide the operator with your passport data or other identification document (driver's license, passport of a citizen of Ukraine for traveling abroad, diplomatic or service passport, seaman's identity card, crew member's identity card, residence permit of the person living in Ukraine , national passport of a foreigner, military identity card, pension identity card); After filling out an application, please contact the nearest Vodafone store.

Plese note! ou can receive a duplicate within 30 days from the date of registration by the operator, take the document indicated in the application.

How to find out the PIN / PUK code?

If You have entered your PIN code incorrectly, most phones are request a PUK code (PUK code specified on the plastic holder), after entering which you enter a new PIN code and the phone is unlocked.

If the plastic holder is not kept, the PUK code can be claryfied in one of the following ways:                            

  • contact an Online-сервисный центр employee;               
  • Call the operator at 111 (free of charge from Vodafone mobile in Ukraine) or 0 (800) 400 000 (for subscribers of other operators (free of charge from any number in Ukraine).                    
  • use the «Internet-Assistant»              

To receive the PUK code You need to confirm that You are the owner of the SIM card, You have to provide a codeword or provide additional information, please:                            

  • Two phone numbers, you recently dialed out or sent SMS to;     
  • the amount on the account and the amount of the last account replenishment or the date of the last action by the number.                    

If the PUK-code has been blocked, You need to fill out an application for the recovery of the number and get a duplicate SIM-card in the Vodafone store


How to block a SIM/USIM-card?

You can block your SIM-card only in case of loss. Contact an operator of the Vodafone Service Center:

0800 400 111 - for subscribers of other operators (free from any number in Ukraine) 
During dialogue with the operator you need to confirm that you are an owner of the card, it is necessary to provide a PUK-code (a code word or answers to questions related to the work of your number as alternative).


If the SIM/USIM card has not been used for a long time?

Each SIM/USIM-card has a validity period. After activation of the card, it makes 60 days (for Vodafone Red XS or Red S, Vodafone Light or Light + starter packs). After each subsequent recharging for 5 UAH (except the Money Transfer service) a period of validity is extended for 365 days. To check the number validity, use the query *101#

If the numbers are expired, the card can be blocked by the operator.
In addition, the card can be blocked in the absence of outgoing events (calls, SMS&MMS, mobile Internet) for more than 180 days. 
If at the end of the validity period the card number is registered in the Vodafone UA network, you get incoming calls but can’t make outgoing ones, to renew the validity period it’s enough to recharge the account in the amount of 5 UAH. Recharging can be made in any way, except the Money Transfer service. 
If the card is not registered in the network, contact an operator of the Vodafone Ukraine Service Center to clarify the possibility to renew the number:

If you lose your SIM/USIM-card, to get a new one in mini-micro-nano format, contact your nearest Vodafone Ukraine Sales and Customer Care Center with your passport.


If the number is blocked by entering an incorrect PIN/PUK code?

If you entered your PIN-code incorrectly, use a PUK-code (PUK-code is indicated on a plastic card in your starter package).
If you enter your PUK-code incorrectly 10 times, your SIM/USIM-card will be blocked. In case of loss or block you PUK-code, please call:

During dialogue with the operator you need to confirm that you are an owner of the card, it is necessary to provide a PUK-code (a code word or answers to questions related to the work of your number as alternative) and leave the passport data for a new USIM-card at the Vodafone Ukraine Sales and Customer Care Centers.

To block your SIM/USIM-card, contact an operator of the Vodafone Service Center:

During dialogue with the operator you need to confirm that you are an owner of the contract subscription:

  • for private individuals: be ready to give your registration data (name, passport series and number, password of the contract, if it is issued);
  • for legal entities: password of the contract and details of the company, with confirmation of the block by application via fax.


If a SIM/USIM-card has not been used for a long time?

If your number was blocked on the debt and you have not used communication services for three months, provision of services may be terminated by the operator. You can contact your nearest Vodafone Ukraine Sales and Customer Care Center with your passport to clarify the possibility to renew your number/card and contract subscription.
